Informed Consent For Ayurveda Services
Welcome to ReConnect, an integrative health center where mind, body & soul heal together. This document contains important information about the Center’s Ayurveda services and business policies. “I”, “Us”, “We”, “Our”, “Ours” and other first-person pronouns will refer to the Center, as well as all employees, contractors, landlords, volunteers, and/or affiliates of the Center. You, as the Client, may be referred to throughout this agreement with second-person pronouns such as “You”, “Your”, “Yours.” This informed consent may be amended at any time by reasonable notice, including without limitation by posting revised content on the center’s website at www.reconnecthealth.com, which amended terms shall be binding upon you.
Ayurveda Services
Ayurveda is a 6000-year-old functional and holistic system of medicine that promotes health, consciousnesses and equilibrium. This Indian traditional system of health focuses on causes not symptoms and has an individual approach to nutrition and lifestyle.
You acknowledge that you have voluntarily chosen to use and/or participate in the Center’s Ayurveda services, which may include Ayurvedic Consultations, Ayurvedic Treatments, and Ayurveda Classes and Workshops (collectively, the “Activities”), which may be offered at the Center’s physical location or offered online via Zoom or another online communication platform.
You may schedule Activities on our website www.reconnecthealth.com or directly via a provided scheduling link. All Activities require online registration with a valid credit card. Ayurvedic consultations and treatments are paid at the time of service. Please provide a 24-hour advance cancellation or reschedule notice otherwise you will be charged for the missed appointment.
Live classes, workshops, and/or other Activities require online registration and full pre-payment. There are no refunds for missed live Activities unless the entire series is canceled 48-hours before the first scheduled session.
Ayurvedic Practitioners or Consultants
You acknowledge that our Ayurvedic practitioners or consultants are not trained in Western medical diagnosis or treatments; they are not physicians, nor licensed health care professionals, but rather educators in Ayurveda. No medical advice, diagnosis or treatment is given by our Ayurvedic consultants; but rather they are providing educational services. If you are suffering from a disease or severe symptoms that have not been evaluated by a medical doctor or another licensed health care professional, we recommend that you seek such licensed professionals for a proper evaluation.
You understand that an Ayurvedic consultation and any assessment or information ensuing thereafter is for general educational purposes only and that no claim to medical diagnosis or treatment, or the cure of any medical condition, is inferred or implied. You further understand that this consultation and any recommendations ensuing thereafter should not be construed as a substitute for medical examination diagnosis and treatment, and that you should see a medical doctor or other licensed health care provider for any physical or mental ailment or complaint that you may have. You understand that an Ayurvedic consultation may include assessment of doshic conditions and related recommendations for dietary, herbal, or other lifestyle regimens and that these are given from a traditional Ayurvedic perspective only. You understand that any suggestions or recommendations are in no fashion intended as a prescription for any condition. You understand that Ayurvedic consultants are not qualified or licensed to diagnose, prescribe for, or treat any physical or mental illness, and that nothing said in the course of any consultation should be construed as such. All the information provided by our Ayurvedic consultants, verbal or written, is for educational purposes only and the information is not to be used as, used as a substitute for, or considered as a medical diagnosis, treatment or prescription. You understand that any healing process requires my active participation and is your own personal responsibility.
You knowingly, voluntarily, irrevocably, and expressly release and waive any claim you may have against the Center, for any injury or damages that you may sustain as a result of participating in the Activities. You hereby release and forever discharge the Center from any liability whatsoever out of or in connection with your participation in any Activities and will further indemnify and hold us harmless from any loss, cost, damage, or expense (including attorney’s fees and cost of litigation) that we may incur in defending any claim made by you or anyone making a claim on your behalf, even if the claim is alleged to or did result from negligence.
Confidentiality
All personal information you share with the Center will be kept strictly confidential unless you consent to sharing it or unless mandated by law. However, if you elect to communicate with the Center by email or cellular text message (SMS), please be aware that email and text messages are not secure, and confidentiality cannot be assured.
You hereby understand that the Center from time to time may photograph, video, or otherwise record Activities occurring at the Center and publish them on its Website or social media platform for marketing purposes. You hereby consent to the use of your image that may appear in any such photograph or video.
Minors
A parent or legal guardian’s authorization is required if the participant is under the age of 18. As a parent or legal guardian of your minor child, you consent to the above terms and conditions in consideration of your minor child’s participation in the Activities.
