Informed Consent For Naturopathic Medicine Services
Welcome to ReConnect, an integrative health center where mind, body, and soul heal together. This document contains important information about the Center’s Naturopathic Medicine services and business policies. “I”, “Us”, “We”, “Our”, “Ours” and other first-person pronouns will refer to the Center, as well as all employees, contractors, landlords, volunteers, and/or affiliates of the Center. You, as the Client, may be referred to throughout this agreement with second-person pronouns such as “You”, “Your”, “Yours.” By clicking on the checkbox below you are agreeing that you have read, understood, and agreed to the content in this document, which will represent an agreement between you and the Center.
Naturopathic Medicine Services
Naturopathic medicine is a system of healthcare that focuses on natural remedies and treatments to support the body’s innate healing abilities. Naturopathic medicine emphasizes prevention and seeks to identify and address the underlying causes of illness, rather than simply treating symptoms.
You acknowledge that you have voluntarily chosen to use and/or participate in the Center’s Naturopathic Medicine services, which may include but are not limited to, a variety of natural therapies, dietary recommendations and nutritional supplements, Botanical/Herbal Medicine, homeopathy, lifestyle counseling, and Naturopathic Medicine Classes and Workshops (collectively, the “Services”), which may be offered at the Center’s physical location or offered online via Zoom or another online communication platform.
You may schedule Services on our website www.reconnecthealth.com or directly via a provided scheduling link. All Services require online registration with a valid credit card. Ayurvedic consultations and treatments are paid at the time of service. Please provide a 24-hour advance cancellation or reschedule notice, otherwise, you will be charged for the missed appointment.
Live classes, workshops, and/or other Services require online registration and full pre-payment. There are no refunds for missed live Services unless the entire series is canceled 48-hours before the first scheduled session.
Naturopathic Doctors (NDs)
You acknowledge and understand Naturopathic Medicine is not currently licensed or recognized as a separate profession in the state of Texas. In Texas, naturopathic doctors (NDs) are not licensed to practice as primary care physicians and cannot legally diagnose or treat medical conditions. No medical advice, diagnosis or treatment is given by our naturopathic doctors. Naturopathic doctors provide educational services. If you are suffering from a disease or severe symptoms that have not been evaluated by a medical doctor or another licensed health care professional, we recommend that you seek such licensed professionals for a proper evaluation.
You understand that a Naturopathic Medicine consultation and any assessment or information ensuing thereafter is for general educational purposes only and that no claim to medical diagnosis or treatment, or the cure of any medical condition, is inferred or implied. You further understand that this consultation and any recommendations ensuing thereafter should not be construed as a substitute for medical examination, diagnosis, and treatment, and that you should see a medical doctor or other licensed health care provider for any physical or mental ailment or complaint that you may have.
You understand and acknowledge that the nature of the Naturopathic Medicine services provided to you may involve certain risks and potential complications, which may include, but are not limited to, allergic reactions to herbal remedies or supplements, interactions with other medications or treatments, aggravation of pre-existing symptoms or conditions, adverse reactions to physical therapies, complications in certain physiological conditions such as pregnancy, lactation, and in certain conditions including but not limited to diabetes, liver, heart, or kidney disease, especially in very young children and very elderly people. It is therefore important to inform your Naturopathic Doctor of any illnesses you suffer from or medications you may be taking (prescription or over-the- counter).
You knowingly, voluntarily, irrevocably, and expressly release and waive any claim you may have against the Center, for any injury or damages that you may sustain as a result of participating in or using the Services. You hereby release and forever discharge the Center from any liability whatsoever out of or in connection with your participation in any Services and will further indemnify and hold us harmless from any loss, cost, damage, or expense (including attorney’s fees and cost of litigation) that we may incur in defending any claim made by you or anyone making a claim on your behalf, even if the claim is alleged to or did result from negligence.
Confidentiality
All personal information you share with the Center will be kept strictly confidential unless you consent to sharing it or unless mandated by law. However, if you elect to communicate with the Center by email or cellular text message (SMS), please be aware that email and text messages are not secure, and confidentiality cannot be assured.
Minors
A parent or legal guardian’s authorization is required if the Client is under the age of 18. As a parent or legal guardian of your minor child, you consent to the above terms and conditions in consideration of your minor child’s participation in or use of the Services.
