Informed Consent for Watsu Hydrotherapy Services

Welcome to ReConnect, an integrative health center where mind, body, and soul heal together. This document contains important information about the Center’s Watsu Hydrotherapy services and business policies. “I”, “Us”, “We”, “Our”, “Ours” and other first-person pronouns will refer to the Center, as well as all employees, contractors, landlords, volunteers, and/or affiliates of the Center. You, as the Client, may be referred to throughout this agreement with second-person pronouns such as “You”, “Your”, “Yours.” This informed consent may be amended at any time by reasonable notice, including without limitation by posting revised content on the center’s website at www.reconnecthealth.com, which amended terms shall be binding upon you. By clicking on the checkbox you are agreeing that you have read, understood, and agreed to the content in this document, which will represent an agreement between you and the Center.

Watsu Hydrotherapy Services

Watsu Hydrotherapy is a mind/body therapeutic aquatic experience incorporating elements of Zen Shiatsu massage, muscle movement, and stretching. Watsu is practiced in a warm, aquatic environment, facilitating deep relaxation and meditation. Watsu is a restorative service which focuses on physical and emotional well-being.

You acknowledge that you have voluntarily chosen to use and/or participate in the Center’s Watsu Hydrotherapy services (collectively, the “Services”), which may include but are not limited to, physical movement, massage, stretching, floating, breathing exercises, and meditation within a heated pool. Watsu involves physical contact with our practitioner and may include submersion in water.

You understand that Watsu Hydrotherapy is for restorative wellness only, and no claim to medical diagnosis or treatment, or the cure of any medical condition, is inferred or implied. You further understand that this service and any recommendations ensuing thereafter should not be construed as a substitute for medical examination, diagnosis, and treatment, and that you should see a medical doctor or other licensed health care provider for any physical or mental ailment or complaint that you may have.

You understand and acknowledge that the nature of the Watsu Hydrotherapy services provided to you may involve certain risks and potential complications, including, but not limited to, risk of injury, risk of psychological discomfort, and risk of contracting illnesses associated with being in the water.

You knowingly, voluntarily, irrevocably, and expressly release and waive any claim you may have against the Center, for any injury or damages that you may sustain as a result of participating in or using the Services. You hereby release and forever discharge the Center from any liability whatsoever out of or in connection with your participation in any Services and will further indemnify and hold us harmless from any loss, cost, damage, or expense (including attorney’s fees and cost of litigation) that we may incur in defending any claim made by you or anyone making a claim on your behalf, even if the claim is alleged to or did result from negligence.

Center’s Policies
All Services require online registration with a valid credit card. Watsu services are paid at the time of service. Please provide a 24-hour advance cancellation or reschedule notice, otherwise, you will be charged for the missed appointment.

Confidentiality
All personal information you share with the Center will be kept strictly confidential unless you consent to sharing it or unless mandated by law. However, if you elect to communicate with the Center by email or cellular text message (SMS), please be aware that email and text messages are not secure, and confidentiality cannot be assured.

Minors
A parent or legal guardian’s authorization is required if the Client is under the age of 18. As a parent or legal guardian of your minor child, you consent to the above terms and conditions in consideration of your minor child’s participation in or use of the Services.
